    Standard Abutment — What Is It?

    A standard abutment is a pre-fabricated, factory-made component seated on an implant. It is available in several angles (0°, 15°, 30°) and fits the implant platform of a given system.

    Indications:

    • Implants in non-esthetic zones (molars)
    • Simple geometry — implant placed in the axis of the planned crown
    • Soft tissue sufficiently thick to cover the abutment margin
    • Patient with a limited budget, where aesthetics is not a priority

    Limitations:

    • No correction of implant angulation beyond available factory options
    • In the esthetic zone, often exposes the metal margin with thin tissue
    • Suboptimal biomechanics with angled implants

    Custom Abutment — When Is It Worth It?

    A custom abutment is milled or printed specifically for a given implant, based on its scan and position. Each abutment is unique.

    Indications:

    • Esthetic zone (incisors, canines) — precision contouring of the implant cervical margin
    • Angulated implant (15° or more from the planned crown axis)
    • Thin soft tissue — risk of metallic margin show-through
    • Patient's aesthetic requirements regarding gingival contour

    Limitations:

    • Cost: 300-500 PLN for a custom abutment (vs 100-200 PLN for standard)
    • Time: 2-4 business days from design approval
    • Logistics: one abutment = one patient, one implant

    Consequences of Poor Choice in the Esthetic Zone

    A standard abutment with thin tissue or an angled implant in the esthetic zone can cause:

    • Metallic shadow (black line) at the gingiva — abutment shows through thin tissue
    • Exposure of the metal margin with gingival recession
    • Tissue inflammation over time — leaky abutment margins

    In esthetic zones, a custom abutment is protection, not a luxury.

    Cost-Benefit in Practice

    Case 1: One implant in a molar, limited budget. Standard abutment — savings of 200-300 PLN, satisfactory result.

    Case 2: Three implants in the esthetic zone. Custom abutments (×3) — cost 900-1500 PLN more, but aesthetics for which the patient pays.

    Case 3: Implant at a 45° angle. Custom abutment — the only option for correct crown axis.
