Implant is Malpositioned – What to Do When the Position Deviates from the Plan

An implant in a non-standard position isn't the end of the world. We have tools. The question is: how significant is the deviation and at what stage did you detect it?

An implant in a non-standard position isn't the end of the world. We have tools. The question is: how significant is the deviation and at what stage did you detect it?

    Why is the implant malpositioned?

The causes vary: anatomy differed from assumptions, patient movement during the procedure, tissue forces necessitated an axis change, or a planning error. In most cases, a deviation of 15-30° from the planned axis can be compensated. Above 30° – options are fewer.

    Angulated solutions

Option 1 – Custom angulated abutment

The first and best option for deviations up to 35°. We mill an abutment that compensates for the implant angle – the crown axis is correct, and the patient sees no difference.

    Parameter Standard Abutment Custom Angulated
Angle Correction 0°, 15°, 30° (factory) Up to 35° custom
Crown Axis Precision Limited Full control
Aesthetics Risk in aesthetic zone Guaranteed
Fabrication Time Ready-made 2-4 business days
    Cost Lower Higher

Option 2 – Factory angulated abutment (prefabricated)

Manufacturers offer abutments in 15° and 30° angles. Faster and cheaper, but limited to available angles. For a 22° deviation – a prefabricated abutment will not fit perfectly.

Option 3 – Crown design modification

We take the implant angle as a starting point and compensate for it with the crown's shape and morphology. This option is for small deviations – up to 10-15°. No additional cost, but with a compromise.

Time window – when did you discover it?

Before crown cementation: full flexibility. Time for a custom abutment: 2-4 days.

After cementation: options are limited. Disassembly destroys the restoration – you start over.

After several months with symptoms (discomfort, inflammation): complicated procedure. The earlier you react, the less work and cost involved.

    Practical tips

    1. Check the position immediately after surgery – do not wait until the prosthetic stage.
    2. If you know the implant angle – provide this information with the order. We adapt the design.
    3. Deviations of 15-30° are common in practice – we routinely work with this.
    4. A custom abutment is an investment that prevents future problems.

Can I seat a standard abutment on an implant at a 45° angle?

You can, but the crown axis will be displaced – visible or dysfunctional. At 45°, a custom angulated abutment is the only sensible option.

How long does it take to fabricate a custom angulated abutment?

2-4 business days from project approval.

Is it the laboratory's fault that the implant is malpositioned?

No – implant position is a result of surgery and planning. The laboratory works with what it receives.
