An implant in a non-standard position isn't the end of the world. We have tools. The question is: how significant is the deviation and at what stage did you detect it?
The causes vary: anatomy differed from assumptions, patient movement during the procedure, tissue forces necessitated an axis change, or a planning error. In most cases, a deviation of 15-30° from the planned axis can be compensated. Above 30° – options are fewer.
The first and best option for deviations up to 35°. We mill an abutment that compensates for the implant angle – the crown axis is correct, and the patient sees no difference.

Manufacturers offer abutments in 15° and 30° angles. Faster and cheaper, but limited to available angles. For a 22° deviation – a prefabricated abutment will not fit perfectly.
We take the implant angle as a starting point and compensate for it with the crown's shape and morphology. This option is for small deviations – up to 10-15°. No additional cost, but with a compromise.
Before crown cementation: full flexibility. Time for a custom abutment: 2-4 days.
After cementation: options are limited. Disassembly destroys the restoration – you start over.
After several months with symptoms (discomfort, inflammation): complicated procedure. The earlier you react, the less work and cost involved.
You can, but the crown axis will be displaced – visible or dysfunctional. At 45°, a custom angulated abutment is the only sensible option.
