    What is a Multi-unit Abutment?

    A multi-unit abutment is an individual component that connects to an implant. Unlike standard abutments (which are straight), a multi-unit can be designed for the specific geometry of the case: angles, positions, multi-variant solutions for multiple implants.

    This is an intermediate solution between an individual abutment on a single implant and bar systems for edentulism.

    Design and Manufacturing

    Multi-unit design process:

    1. 3D scan of the patient — each implant has its position
    2. CAD design — each multi-unit is unique
    3. Milling or printing — PMMA, zirconia, or titanium materials
    4. Fit verification — each component must fit passively
    5. Mounting on implants — in clinical practice

    Turnaround time: 7-10 business days from design approval. Cost: higher than standard abutments, comparable to individual abutments × number of implants.

    Advantages and Disadvantages

    Advantages:

    • Maximum control over the geometry of each component
    • Adaptability to any patient anatomy
    • Flexibility — various components can be combined (bar, overdenture, even partially fixed)

    Disadvantages:

    • Higher cost
    • Longer turnaround time
    • Requires advanced CAD software
    • Lack of universal components — each part is unique

    Practical Cases

    Case 1: Edentulism on 4 implants, complex anatomy The patient has implants at different angles (two straight, two at 40°). A standard bar system does not fit. Solution: 4 multi-unit abutments + a periodontal structure + prosthetic work. Higher cost, but ideal ergonomics.

    Case 2: Patient wants an overdenture, but anatomy does not allow for a Locator Patient's anatomy (asymmetrical resorption, closely spaced implants) rules out a standard system. Solution: design individual "overdenture connectors"—an intermediate solution between standard and custom.

    Alternatives to Consider

    Before proposing a multi-unit:

    • Bar system — cheaper, faster, proven. If anatomy allows — preferred
    • Standard abutments + overdenture — simplest solution for most cases
    • Fixed prosthesis on multiple implants — if the patient agrees to the cost and requirements
